Key Responsibilities
Conduct telephonic outreach to engage members
Complete assessments to identify member needs
Maintain member records and documentation
Conduct follow-up calls to ensure member needs are met
Provide information about care management services
Provide administrative support to medical and disease management programs
Act as a liaison and track referrals to care management programs
Assist in coordinating care for high-risk patient populations
Verify health plan benefits and coordinate ambulatory services
Precept new hires and assist in auditing efforts
Required Qualifications
High school diploma
3 years of relevant experience
1 year experience in a managed care environment
Fluent in English and Spanish
Preferred Qualifications
Health insurance/managed care experience (Commercial, Medicare, and Medi-Cal)
Knowledge of community resources and advocacy
Ability to identify issues and develop effective solutions
