Job Description
The Case Manager at The Jewish Board supports adults with serious mental illness in achieving independence. This senior role involves managing complex cases, advocating for residents, and facilitating their recovery goals through personalized support and collaboration with other service providers.
Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning with residents
Evaluates and assesses independent living skills and supports residents in meeting individual goals
Advocates on behalf of residents with other service providers
Completes documentation of all resident interactions, including assessments and service plans
Participates in planning and execution of special and holiday events
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van
