Required Qualifications
PharmD Pharmacy
Bachelor's Degree BS in Pharmacy
Oregon Pharmacist License upon hire
2 years PGY2 residency training or PGY1 residency training and 2 years of experience in relevant practice setting or a minimum of 4 years of experience in specialty practice setting
Preferred Qualifications
BPS Board Certified Pharmacist within 2 years of hire
