Key Responsibilities
Provide patients with diagnosis, treatment, and care under the supervision of a physician
Focus on providing palliative care

Required Qualifications
Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
2 years experience within the last 5 years in chronic disease management/long-term care
Reliable transportation
Ability to sit, stand, bend, move intermittently and lift at least 25lbs
