The Access Center RN Coordinator manages and monitors complex hospice and palliative care referrals to ensure timely admissions into hospice programs. They coordinate communication between multidisciplinary hospice teams, including nurses, physicians, and social workers, to verify and facilitate safe patient admissions. The role requires nursing licensure, hospice experience, and strong communication and customer service skills to support patients and families dealing with life-limiting conditions.

Reporting to the Access Nurse Team Leader, you will be responsible for managing high-tech and complex referrals in the Access Center. You will monitor the Access referral list and work with the Access Coordinators to ensure that referrals are admitted on to the Hospice program in a timely fashion.
