Job Description

Join our compassionate team as a Nurse Navigator, providing essential care coordination for patients with serious illnesses. You'll ensure high-quality care throughout their healthcare journey while working collaboratively with families, medical practices, and our dedicated team.

Key Responsibilities

  • Provide comprehensive care coordination and navigation services to patients with serious illnesses
  • Respond promptly to clinical inquiries and document interactions in EMR
  • Conduct in-home and telehealth assessments and provide ongoing care navigation
  • Educate patients, families, and referral sources about agency programs
  • Complete necessary paperwork and administrative documentation
  • Coordinate with clinical and administrative team members
  • Monitor EMR and other program channels for patient needs
  • Ensure accurate and complete information in patients' insurance records
  • Represent the agency positively with patients and families

Required Qualifications

  • Active State of Maryland or Compact RN license in good standing
  • 2-3 years of nursing experience in medical/surgical, critical care, home care, hospice or palliative care setting
  • Associate’s degree in nursing or related field
  • Excellent nursing assessment and verbal presentation skills
  • Time management and high-quality organizational skills
  • Proficient in Microsoft Office Products and EMRs
  • Current Maryland driver’s license and clean driving record
  • Possess or access to reliable transportation

Preferred Qualifications

  • BSN
  • Certification in Hospice and Palliative Care Nursing (CHPN) preferred and required within two years of employment
  • Enthusiasm for nursing and passion for caring for patients with serious illness
