Join The Jewish Board as a Case Planner and make a significant impact by supporting at-risk families in their homes. You'll provide essential counseling and advocacy services to ensure children's safety, while collaborating with various community resources to foster family stability.
Key Responsibilities
Provide Child Welfare Preventive Services in the client's home and community
Assess and address safety and risk concerns related to children's health and well-being
Evaluate living conditions of clients through home visits
Provide supportive case work counseling to children and family members
Collaborate with external resources in the family's ecology
Conduct specialized groups for children, adolescents, and parents
Complete weekly progress notes and enter data into PROMIS
Carry caseload size as identified by program service model
Monitor children regarding their medical needs
Participate in Family Team Conferences and internal quality improvement projects
Required Qualifications
BA or Masters level degree in Psychology, Social Work, Counseling or a related subject
Basic social work knowledge and understanding of stages of change
Ability to provide in-home child welfare services
Ability to establish rapport and therapeutic relationship with families
Preferred Qualifications
Understanding of the use of family therapy principles to provide services
Understanding of early childhood development and related theories and models of practice
Strong Assessment skills to evaluate dynamics and interactions within the family
Knowledge and experience with the Connections system
