Job Description

Key Responsibilities
Conduct adult/geriatric health assessments
Administer and order point of care and lab-based testing
Collaborate with healthcare professionals for patient care
Document health assessments according to guidelines
Provide services at patients' residences
Required Qualifications
Master’s Degree or commensurate experience
Current NP or PA licensure with prescription authority
Board certified in relevant specialty
Current BLS, ACLS or CPR certification
Valid state driver's license
Preferred Qualifications
1 year experience as a Nurse Practitioner or Physician Assistant
Strong computer skills
Excellent verbal and written communication skills
