Nationwide Therapy Group is seeking a travel Pharmacist for a travel job in Phoenix, Arizona.
Job Description & Requirements
- Specialty: Pharmacist

- Shift: 8 hours, days
- Employment Type: Travel
- Medication Management: Procurement, dispensing, sterile compounding, parenteral prep/labeling, controlled substances accountability, formulary maintenance.
- Clinical Services:
- Antimicrobial stewardship (interpret cultures, UTI, pneumonia, SSTI guidance).
- Pharmacokinetic dosing & monitoring.
- Anticoagulation dosing & monitoring.
- Medication reconciliation (admission & discharge), including discharge counseling.
- ADR monitoring and reporting.
- Interdisciplinary Support: Participate in P&T Committee, patient drug therapy monitoring, and provider consultations.
- Strong clinical knowledge (antimicrobial stewardship, pharmacokinetics, anticoagulation).
Requirements:
Current state pharmacist license.
BLS (if required by state).
EPIC experience required.
