Key Responsibilities
Provide and direct nursing and personal care to patients in their homes
Coordinate the plan of care with other healthcare professionals
Complete clinical nursing assessments
Develop and revise individualized plans of care
Ensure patient eligibility and medical necessity for services
Required Qualifications
Current RN licensure in state of practice
Minimum of one year experience as an RN
Current CPR certification
