Key Responsibilities
Function as a case planner and ensure client care
Develop action plans for client resources
Conduct routine community contacts and evaluations
Perform home visits and assess living conditions
Provide individual/group counseling
Facilitate client referrals to treatment or service providers
Perform community outreach and establish linkages
Maintain documentation and progress notes
Required Qualifications
BA Degree with experience in community or social service group
At least five years of experience as a Case Assistant
