Job Description

Join Optum as a Per Diem Nurse Practitioner or Physician Assistant, where you will play a crucial role in supporting patients transitioning from rehab to home, ensuring their health needs are met with compassion and expertise.

Key Responsibilities

  • Perform comprehensive assessments and document findings
  • Ensure all diagnoses are ICD10 coded accurately
  • Address and document all quality elements
  • Conduct initial medication reviews
  • Travel between care sites as required
  • Communicate with the patient and necessary team members regarding medical concerns

Required Qualifications

  • Certified Nurse Practitioner or Physician Assistant
  • Graduate of an accredited master's degree in Nursing or Physician Assistant program
  • Active and unrestricted license in the state of residence
  • Current active DEA licensure/prescriptive authority
  • Access to reliable transportation
  • Ability to lift a 30-pound bag and navigate stairs
  • Availability to work 24 hours per month

Preferred Qualifications

  • 1+ years of hands-on post grad experience within Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including electronic medical records
  • Ability to develop and maintain positive customer relationships
  • Adaptability to change
