Key Responsibilities
Utilize patient information sources and communication with the care team to identify patients needing acute inpatient rehabilitation or LTACH
Review patients daily until clinical evidence supports a physician order for pre-admission assessment for rehab or LTACH
Collaborate with assigned case manager regarding patient assessments
Required Qualifications
Licensed Professional in a clinical health care specialty (RN, Physical Therapist, Occupational Therapist, Speech Therapist)
Graduate of an accredited school in clinical specialty
Current state licensure in clinical practice area or eligible for state license transfer
Minimum 2 years recent clinical care experience
Effective verbal, written and persuasive communication skills
Computer and keyboarding skills
Ability to work with others to ensure quality patient care
Organization and time management skills
Strong problem solving skills
Strong interpersonal skills
Preferred Qualifications
Speech Pathologist, Registered Nurse, Occupational Therapy (OT), or Licensed Physical Therapist must be obtained within 30 days of employment start date
Bachelor's Degree, Doctorate Degree, or Master's Degree
