Speech-Language Pathologist (SLP) – Skilled Nursing Facility (SNF)
Location: Milford, DE

MedAdventures is seeking a skilled and compassionate Speech-Language Pathologist (SLP) to join a well-established Skilled Nursing Facility (SNF) team in Milford, DE. This is an excellent opportunity for an SLP who is passionate about helping residents regain and maintain their communication and swallowing abilities, ultimately improving their quality of life.

In this role, you’ll work collaboratively with an interdisciplinary care team to evaluate, treat, and support residents with diverse speech, language, cognitive-communication, and swallowing needs.

Key Responsibilities:

• Evaluate residents and develop individualized treatment plans for speech, language, cognitive, and swallowing disorders
• Provide skilled therapy services in accordance with physician orders and care plans
• Collaborate with PTs, OTs, nursing staff, and families to ensure coordinated care
• Educate patients and caregivers on communication strategies and safe swallowing techniques
• Accurately document evaluations, progress notes, and discharge summaries

Qualifications:

• Active Delaware SLP license (or eligibility to obtain)
• Experience in a skilled nursing or geriatric setting preferred
• Strong clinical reasoning, documentation, and communication skills
• Passion for patient-centered care and functional outcomes
