Key Responsibilities
Provide general medical care and care coordination to various patient levels
Perform comprehensive evaluations and preventative assessments
Address chronic and acute primary care complaints
Establish and document medical diagnoses
Order and perform diagnostic testing
Create and implement medical plans of care
Schedule patient appointments
Provide post-discharge coordination
Perform face-to-face visits in various settings
Participate in community-based clinics
Drive to various locations regularly
Collaborate with fellow nurse practitioners
Prescribe medications and perform procedures
Document in electronic medical records
Required Qualifications
Master's degree in family health from accredited nursing program
Active and unrestricted national certification from a recognized organization
Current state-issued license to practice as a Family Nurse Practitioner
Current Basic Life Support for Healthcare Professional certification
Current unrestricted driver’s license
Advanced computer skills including proficiency with Word, Excel, and Electronic Medical Records
Preferred Qualifications
3-5 years of experience as a Registered Nurse and/or Nurse Practitioner
Experience in home health, community health, or public health setting
