Job Description

Join UnitedHealthcare as a Field Care Coordinator in Michigan, where you will play a vital role in enhancing healthcare access and quality for members. This position involves developing personalized care plans, advocating for patients, and coordinating with healthcare teams to ensure effective service delivery, all while working primarily from home with some travel.

Key Responsibilities

  • Develop and implement care plan interventions
  • Communicate health-related information to stakeholders
  • Advocate for patients and families
  • Assess, plan, and implement individualized care strategies
  • Identify barriers to care and provide interventions
  • Initiate referrals for social service programs
  • Provide resource support for local services
  • Manage the person-centered service/support plan
  • Conduct home and in-person visits
  • Document member information and care management activities

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan
  • Master’s degree and current, unrestricted independent licensure as a Social Worker (e.g., LMSW, LCSW, LLMSW)
  • Bachelor’s degree and current, unrestricted independent licensure as a Social Worker (e.g. LLBSW, LBSW)
  • 2+ years of experience in community health setting
  • 1+ years of experience with local behavioral health providers
  • 1+ years of experience with long-term care needs
  • 1+ years of experience with electronic documentation systems and MS Office
  • Access to reliable transportation for travel up to 75%
  • Access to a designated quiet workspace at home
  • Ability to travel to Southfield, MI for quarterly meetings
  • Must reside in Michigan

Preferred Qualifications

  • RN or LMSW, LLMSW, LCSW
  • 1+ years of medical case management experience
  • Experience in Motivational Interviewing, Stages of Change, Trauma-Informed Care
  • Experience with co-occurring disorders
  • Experience with MI Health Link (MMP)
  • Experience in Managed Care
  • Knowledge of NCQA documentation standards
