Key Responsibilities
Direct case management of assigned clients
Plan, implement, and evaluate care provided
Collaborate with regional director and personnel to improve services
Serve as a clinical resource and ensure clients' home care needs are met
Participate in staffing and scheduling discussions
Required Qualifications
Graduate of a state approved school of professional registered nursing
Current, unrestricted RN license in the state(s) of practice
Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
Current CPR certification
Preferred Qualifications
BSN preferred
One (1) year of supervisory and/or case management experience preferred
