Key Responsibilities
Link members to necessary health services
Assess risk and needs, develop care plans

Required Qualifications
Bachelor's degree in social work, psychology, nursing, or related field
Two years of experience in direct services or case management for mentally disabled or chronically ill individuals
Experience working in interdisciplinary teams
Familiarity with Electronic Health Records (EHR)
Preferred Qualifications
Fluency in a second language (Spanish, Russian, or Creole)
Experience in care management or care coordination in a medical or behavioral health environment
