Job Description
Join CenterWell Home Health as a Licensed Practical Nurse and make a meaningful impact by providing personalized care to patients in their homes. Collaborate with a dedicated healthcare team to help patients regain independence and improve their quality of life.
Key Responsibilities
Provide professional nursing services under the direction of an RN
Utilize a holistic approach in the provision of therapeutic interventions
Confer with the RN Supervisor regarding changes in the Plan of Treatment
Maintain contact with patients and healthcare team members regarding patient needs
Prepare, submit and maintain required documentation
Required Qualifications
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
Current nursing license in the practicing state
Valid driver's license and auto insurance
Current CPR certification
Two years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
