Job Description

Join our Community Health Worker Training Program and gain the essential skills to serve as a vital link between healthcare and underserved communities. This temporary program includes both online and hands-on training, culminating in a stipend of $7,500 upon completion.

Key Responsibilities

  • Establish trusting relationships with patients and their families
  • Coach patients in effective management of their chronic health conditions
  • Assist patients in understanding care plans and instructions
  • Advocate for patients and act as a liaison between patients and community service agencies
  • Facilitate communication and coordinate services between providers
  • Document activities, service plans, and results effectively

Required Qualifications

  • Must be 18 years or older
  • Must have a high school diploma or GED
  • Must be a U.S. Citizen or Permanent Resident/Green Card holder
  • Must be able to pass a national criminal background check

Preferred Qualifications

  • Familiarity with Microsoft Office Suite, particularly Word, Excel, and PowerPoint
