Key Responsibilities
Establish trusting relationships with patients and their families
Coach patients in effective management of their chronic health conditions
Assist patients in understanding care plans and instructions
Advocate for patients and act as a liaison between patients and community service agencies
Facilitate communication and coordinate services between providers
Document activities, service plans, and results effectively
Required Qualifications
Must be 18 years or older
Must have a high school diploma or GED
Must be a U.S. Citizen or Permanent Resident/Green Card holder
Must be able to pass a national criminal background check
Preferred Qualifications
Familiarity with Microsoft Office Suite, particularly Word, Excel, and PowerPoint
