Job Description
Join our team as an MDS Coordinator / Nurse Assessment Coordinator, where you will play a crucial role in ensuring exceptional resident care through accurate assessments. Key Responsibilities
Complete and assure the accuracy of MDS assessments for all residents
Determine Patient Driven Payment Method (PDPM) and related expenses
Participate in admitting prospective residents and assessing nursing needs
Monitor Case Mix Index (CMI) scores
Ensure compliance with state and federal regulations
Collaborate with interdisciplinary teams
Provide education to facility staff and leaders
Required Qualifications
Valid state RN nursing license
Direct care in a long-term care setting
Strong organizational skills
Excellent written and verbal communication
Ability to work in a multidisciplinary team
Preferred Qualifications
Advanced degree or certification
MDS Coordinator experience
Clinical Reimbursement Specialist experience
Knowledge of MDS, EMR, PDP, MDS 3.0, Medicaid and Medicare requirements
