Key Responsibilities
Provide direct clinical services to patients in the Collaborative Care Model (CoCM)
Screen and assess patients for common mental health and substance abuse disorders
Track treatment response and monitor patients for changes in clinical symptoms
Support psychotropic medication management
Provide brief behavioral interventions using evidence-based techniques
Participate in caseload consultation with psychiatric consultants
Develop and complete relapse prevention self-management plans
Required Qualifications
Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Advanced Practice Social Worker (LAPSW), Licensed Mental Health Counselor (LMHC), Licensed Professional Counselor - MHSP (LPC-MHSP), or Licensed Marriage and Family Therapist (LMFT)
3+ years of experience treating patients with behavioral health conditions
1+ years of experience working in primary care or other medical behavioral integrated settings
Experience with assessment and using evidence-based psychosocial treatments and brief behavioral interventions
Currently eligible to be enrolled as a provider in the Medicare program
Preferred Qualifications
Experience in the Collaborative Care Model (CoCM)
Detail-oriented and organized with exceptional follow-up capabilities
Ability to maintain effective and professional relationships with patients and care team members
Experience working with patients in person and via telehealth
Solid grasp of technology solutions and tools
