Key Responsibilities
Administers medications and implements primary care orders
Evaluates participant complaints physically and/or by telephone/telehealth
Provides education to participants and caregivers regarding care plans and medications
Maintains participants’ medical records
Observes and reports participant’s condition and reactions to treatments
Participates in planning and executing interventions with interdisciplinary team
Assists in scheduling specialist appointments and hospital admissions
Required Qualifications
Graduate of an accredited school of nursing
Current LPN/LVN license in good standing
BLS certification (must have within 90 days of employment)
Preferred Qualifications
One year working as an LPN preferred
IV Certification preferred
1 year of experience working with a frail or elderly population preferred
