Opportunity Overview:
  • Location: Grand Junction, CO
  • 15 -20 patients per day
  • Flexible schedule Monday to Friday 2 days a week
  • Day call on coverage days only
  • Post-acute care inpatient setting
Responsibilities:
  • Drive high-quality, safe, and cost-effective patient care in post-acute care communities
  • Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
  • Work closely with hospice, palliative care, and home care agencies for safe patient transitions
  • Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
  • Develop and implement individualized care plans
  • Ensure residents receive high quality medical care and oversight
Qualifications:
  • Board Certified internal medicine (IM) or family medicine (FM)
  • Active Colorado license
  • Active DEA
Compensation
Uncapped earning potential: our compensation is a productivity-based model (fee for service); expected compensation is estimated salary range of $108,000 to $130,000 annually with no cap on productivity income potential with full benefits.
