Job Description

Key Responsibilities
Conduct Adult/Geriatric assessments including medical history, diagnosis, treatment, and health education
Work collaboratively with physicians, case managers, and social workers
Provide follow-up and patient education in collaboration with Primary Care Physicians
Conduct services in various venues including home visits and skilled facility visits
Required Qualifications
Master’s Degree or commensurate experience
Current RN and NP licensure in state of practice
Board certified by AANP, ANCC or AACCN
Current BLS, ACLS or CPR certification
1 year experience as a Nurse Practitioner preferred
