Talent4Health is seeking a travel nurse RN Case Management for a travel nursing job in Bronx, New York. Specialty: Case Management - Discipline: RN - Shift: 8 hours, days - Employment Type: Travel Case Management Department – Unit Notes Minimum 1 year of inpatient case management experience required - Prior critical care experience preferred - PRI & SCREEN Certifications – Strongly preferred- Can be completed online through the School of Albany - Focused on discharge planning; Work collaboratively with interdisciplinary teams, including physicians and social workers - Expected to round on the unit with the care team - Make referrals and assist with discharge to home, community settings, or SNFs - May help with:- Housing placement and Shelter Packets (for patients experiencing homelessness) - Typically covers Medical-Surgical (Med-Surg) or Telemetry units - Patient load varies by unit setup:- Standard Floor: 1 CM & 1 SW | Caseload: 17–25 patients - Pilot Floor: 1 CM & 2 SWs | Caseload: 12–17 patients - Patient population includes those under Internal Medicine and Family Medicine UR Software (Preferred): CarePort, MCG, InterQual Inpatient units only - Schedule & Shift Info - Shift Hours: Monday–Friday, 8:00 AM – 4:30 PM- (7.5-hour shifts with a 1-hour unpaid lunch break) - Weekend Coverage: Rare, with manager approval- Weekend shifts may involve covering multiple units - Holiday Coverage: Rare, with manager approval Business casual or scrubs (except Royal Blue or Maroon)- Neutral-colored scrubs preferred - Lab Coat: Provided by the facility Hospital Orientation: 2 days (includes EMR training) - Unit-Specific Training: 1–2 weeks with a preceptor- Duration depends on individual progress A leader and innovator, offering comprehensive talent solutions for the healthcare industry across the nation.
