Join Interim HealthCare as a Licensed Practical Nurse (LPN) and enjoy the flexibility of visiting patients in their home environments. You'll provide essential care coordination, skilled wound care, and patient education, all while being part of a supportive interdisciplinary team.
Key Responsibilities
Participate in ongoing interdisciplinary assessment of the patient
Provide services ordered by the physician as indicated in the plan of care
Recognize and report changes in the patient’s condition to the RN
Counsel and educate the patient and family in healthcare-related needs
Complete timely and accurate clinical notes addressing patient’s progress
Required Qualifications
Current license as a Licensed Practical Nurse (LPN) in the State of MN
2 years experience as an LPN
Valid driver's license and reliable transportation
CPR Certification
