Key Responsibilities

  • Provides basic nursing care by assisting in the assessment of the resident
  • Communicates significant changes in condition to the interdisciplinary team
  • Accurately documents all assessments and interventions
  • Participates in the care planning process for residents
  • Implements necessary nursing interventions relevant to the plan of care
  • Monitors the care of residents closely
  • Identifies problem areas and uses the QAPI process for resolution
  • Answers call lights and assists with residents' ADLs as needed

Required Qualifications

  • Education from an accredited school of nursing that results in becoming a Licensed Practical Nurse
  • Current LPN license in the state you will be working
  • CPR and AED certification required, or willing to obtain within six months of employment
  • IV certification required, or willing to obtain within six months of employment
  • Ability to assess the basic nursing care needs of the geriatric resident
  • Excellent communication skills
  • Time management and organizational skills
  • Ability to stand and walk for long periods of time
  • Ability to lift heavy objects using proper lifting techniques
  • Ability to transfer and move patients who vary with ability to assist weighing 200+lbs
  • Ability to push and pull equipment weighing in excess of 40lbs
  • Manual dexterity, coordination and skillful use of hands
  • Visual acuity with the ability to distinguish color
  • Ability to physically perform necessary documentation electronically and in writing

Preferred Qualifications

  • Long-term care experience
