Job Description

Key Responsibilities
Assess referred patients requiring rehabilitation services
Provide direct patient care and assess medical conditions, functional capabilities, and rehabilitation potential
Establish and administer treatment programs with specific goals
Document and record patient conditions
Educate patients and families on physical therapy methods
Develop a plan of care for each patient
Provide skilled physical therapy services according to physician orders
Evaluate and modify care to meet therapy goals
Collaborate with disciplines to evaluate team goals
Required Qualifications
Doctorate degree in Physical Therapy OR Bachelor's degree in Physical Therapy
Passed the NPTE
Graduated from an accredited school
Minimum one year's experience
Current state license as a Registered Physical Therapist
Current CPR certification
