Key Responsibilities
Assist in home care coordination functions
Explain service options and oversee referrals
Contact agencies and deliver equipment as needed
Enter data into patient electronic medical record systems
Participate in rounding
Perform various office duties including supply ordering and customer service
Required Qualifications
High school diploma or equivalent
One year data entry experience
One year customer service experience in a health care setting
Ability to lift and carry medical equipment
Attention to detail
Ability to work weekends and extended shifts as needed
Preferred Qualifications
Post high school education or training
Previous scheduling experience
