Key Responsibilities
Make home visits to clients and conduct Start of Care (Admissions) visits
Perform diagnostic tests and measurements
Develop and implement individualized care plans
Continually assess and revise the Physical Therapy care plan
Educate clients and family members on rehabilitative care
Document observations and interventions accurately
Required Qualifications
Current New Jersey Physical Therapist License
Minimum of one year of recent work experience as a Physical Therapist
OASIS experience
Preferred Qualifications
Experience with HCHB
