Job Description
The Case Manager at The Jewish Board supports adults with serious mental illness to live independently in the community. This role involves providing personalized support, advocating for residents, and ensuring they achieve their recovery goals in a compassionate and culturally competent manner.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Supports residents in meeting individual goals and medication management independence
Advocates on behalf of residents with other service providers
Completes documentation of all resident interactions
Links residents to community-based treatment and collaborates with service providers
Participates in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
