Job Description
Join The Jewish Board as a Case Manager and make a significant impact on the lives of adults with serious mental illness. In this role, you will support residents in achieving independence through personalized recovery goals while advocating for their needs in a collaborative environment.
Key Responsibilities
Carries a complex caseload and practices person-centered service planning
Evaluates independent living skills and supports residents in meeting individual goals
Advocates on behalf of residents with service providers
Completes documentation of resident interactions and progress
Participates in intake, assessments, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
