Job Description
Join The Jewish Board as a Case Associate, where you will have the opportunity to make a significant impact in the lives of New Yorkers by providing essential case management and support services. You will work within the community to develop action plans, connect clients with resources, and ensure comprehensive care.
Key Responsibilities
Function as a Case Planner to ensure client care
Develop action plans to utilize existing and new resources for client care
Conduct routine community contacts and home visits
Evaluate living conditions and assess independent living skills of clients
Run groups and provide case management
Perform community outreach and establish linkages with other organizations
Provide necessary documentation such as notes and progress reports
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Spanish
Bilingual Haitian-Creole
