Job Description
Join Eagle's Trace as the Memory Care Manager, overseeing the Memory Care neighborhood and integrating Memory Care services across Skilled Nursing and Assisted Living. You'll ensure high-quality care and support for residents, families, and staff, fostering a vibrant community.
Key Responsibilities
Conducting pre-residency assessments and screening of potential residents
Evaluating the psychosocial and activity of daily living (ADL) needs of residents
Monitoring engagement of activities for residents with cognitive impairment
Leading a holistic, Person-Centered approach to assessment and care planning
Partnering with other departments to develop support groups and educational programming opportunities
Supporting Independent Living and Post-Acute Social Workers
Ensuring compliance with Federal, State, and local regulations
Required Qualifications
Minimum of 3 years related experience and/or training in long term care/assisted living
Minimum of 3 years related experience in providing services for residents with cognitive impairment
Preferred Qualifications
Current CPR certification
Knowledge of the geriatric population and the aging process
Demonstration of progressive approaches to supporting and enhancing the quality of life for seniors with cognitive impairment
