Key Responsibilities
Provide a full range of advanced clinical home care services, including patient assessment, medication management, and patient/family education
Coordinate, implement, and continually evaluate the home care needs of patients
Review patient insurance information and coordinate activities involved in each individual care plan
Required Qualifications
Current NYS RN license and registration
Associate's degree in nursing
Minimum of one year acute medical-surgical nursing experience
