Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Collaborate with interdisciplinary teams for care coordination
Utilize therapeutic methods in accordance with care plans
Monitor and revise patient care plans as needed
Provide direct patient care including counseling and emotional support
Maintain timely patient-centered documentation
Participate in administrative and development activities
Required Qualifications
Master's Degree in Social Work, Counseling, or Marriage and Family Therapy
LCSW, LPC, LMFT, LADC licensure
Previous experience in a healthcare setting
Valid Driver's License
Computer proficiency
Excellent communication skills
Preferred Qualifications
Experience with interdisciplinary teams
Knowledge of health-related social needs and cultural competence
Ability to provide clinical social work interventions to diverse patient populations
