Job Description
Join Sunshine State Health Plan as a Case Manager and be a vital part of transforming healthcare for our 28 million members. This role involves developing personalized care plans and collaborating with providers to ensure members receive high-quality care tailored to their needs.
Key Responsibilities
Develop, assess, and facilitate complex care management activities
Evaluate member needs and barriers to care
Develop ongoing care/service plans
Coordinate between members and care provider teams
Monitor member status and revise care plans as necessary
Provide resource support for local services
Facilitate care management with providers
Perform outreach to assess member needs
Document member information and care management activities
Educate members and families on healthcare options and benefits
Provide feedback to leadership on care improvements
Required Qualifications
Degree from an Accredited School of Nursing
Bachelor's degree in Nursing
2-4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State Licensure
Preferred Qualifications
Minimum of two years of pediatric experience
Willingness to travel up to 80% locally
