Key Responsibilities
Develop and facilitate complex care management for mental and behavioral health needs
Evaluate member needs and recommend care plans
Coordinate with providers and community resources
Monitor care plans and member outcomes
Educate members and families on healthcare options
Provide feedback for care enhancement
Required Qualifications
Master's degree in Behavioral Health or Social Work
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN
2 – 4 years of related experience
Pediatric care experience
Preferred Qualifications
Experience with mental health and substance use disorders
Ability to perform telephonic and in-home visits
