Key Responsibilities
Evaluate patients for functional limitations and develop personalized therapy plans
Provide therapy services to treat medical and surgical conditions
Instruct and supervise patients and families on home exercise programs
Continuously assess the patient's condition and adjust therapy plans
Document clinical evaluations, treatments, and patient responses to therapy
Required Qualifications
NY Professional License and current registration as a Physical Therapist
Minimum 1 year of full-time experience providing physical therapy services
Ability to assess and respond to patient and family needs in a home care setting
Valid driver's license and access to a reliable automobile for travel
Preferred Qualifications
Home care experience
Ability to use technology for patient documentation and care
Experience with Oasis assessments
