Key Responsibilities
Evaluate and establish a plan of care for patients referred for Hospice Care services
Facilitate admission of referrals upon discharge from the hospital
Develop and maintain positive relationships with healthcare professionals
Required Qualifications
Licensed Registered Professional Nurse in NYS
Minimum of two years experience in Hospice nursing
Proficiency in physical assessment and interviewing skills
Client-service focused
Excellent written and verbal communication skills
Advanced critical thinking and problem solving skills
Ability to work independently
Preferred Qualifications
Bachelor's Degree
Bilingual Spanish/English (required in some locations)
