Job Description
Become a Host Home / Shared Living provider with Mosaic at Home® in Iowa. You will share your home with an individual with a disability, providing support and mentoring while fostering their independence and quality of life.
Key Responsibilities
Maintain a safe and healthy home environment
Assist with daily living tasks
Provide support for cooking, cleaning, laundry, and budgeting
Medication administration
Support with health and well-being
Daily documentation via web-based case management application
Provide community inclusion and socialization experiences
Compliance with state/agency policies
Communicate effectively with team members
Provide transportation
Required Qualifications
Must reside in Iowa
Transportation and valid driver's license
Active home/renters insurance
Own or rent a home/apartment/condo/townhouse
At least 18 years of age
Reliable computer and internet access
Complete all state mandated trainings
Ability to pass State/Federal and APS/CPS background check
Preferred Qualifications
Experience in caregiving preferred but not required
