As a Care Coordinator at The Jewish Board, you will play a crucial role in connecting individuals with chronic behavioral health and medical conditions to essential services. Working primarily in the field, you will develop personalized care plans, manage care services, and ensure members access the support they need to optimize their health outcomes.
Key Responsibilities
Link adults and children with chronic behavioral health and medical conditions to necessary services
Develop Individualized Care Plans (ICPs) in collaboration with members and their families
Provide care management services and track appointments
Conduct periodic assessments of members' medical and behavioral health needs
Provide outreach services and respond to member inquiries
Complete required documentation and share knowledge with team members
Required Qualifications
Bachelor's degree in social work, psychology, nursing, or related field
Two years of experience in providing direct services or case management to individuals with complex social or healthcare needs
Preferred Qualifications
Experience in interdisciplinary teams
Experience providing care management or coordination in a medical or behavioral health environment
Fluency in a second language such as Spanish, Russian, or Creole
