Key Responsibilities
Assess the patient’s social and emotional state related to illness or injury
Evaluate relationships of medical and nursing needs in the home setting
Obtain community resources to assist in patient recovery
Instruct patients and families on coping with social and emotional responses
Assist in the development and re-evaluation of the physician's Plan of Care
Observe, record, and report changes in patients’ condition
Participate in care coordination activities as part of the interdisciplinary team
Supervise and evaluate the performance of the Social Work Assistant
Maintain and submit required documentation
Required Qualifications
Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education
Social Worker licensure in the state of practice
Valid driver’s license, auto insurance, and reliable transportation
Proof of current CPR certification
Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice
Knowledge of discharge planning needs and community resources
Excellent oral and written communication and interpersonal skills
