Position Summary
The RN – Wound Care is responsible for conducting health assessments focused on wound, ostomy, and continence care. This includes performing focused physical assessments, developing individualized care plans, recommending appropriate treatments, and collaborating with the healthcare team to ensure high-quality patient outcomes.
Key Responsibilities
- Develop, revise, and evaluate standards of care
- Assist in selecting supplies, technology, and equipment
- Assess and document patient responses to wound and ostomy treatments
- Evaluate care delivery by clinical staff
- Collaborate with interdisciplinary teams and make referrals as needed
- Educate and counsel patients and families on prevention and care
- Develop and deliver staff education programs
- Conduct comprehensive assessments for pressure injury prevention
- Escalate and communicate challenges, including ethical and conflict resolution issues
- Collaborate with the Assistant Director of Nursing for Wound Care to recommend best practices
- Participate in unit and hospital-wide quality improvement initiatives
- Provide culturally competent education to patients, families, and staff
- Maintain confidentiality and data security
- Engage in professional development activities
- Identify and refer patients to appropriate community resources
- Manage assets related to rented equipment (e.g., specialty beds, negative pressure devices)
Required Skills & Experience
- Minimum of 2 years of medical/surgical experience
- Experience in wound and colostomy care
Education & Licensure
Required Education:
- Associate Degree in Nursing (ASN, ADN) from an accredited program
Required Licensure & Certifications:
- Active RN license in New York State
- Primary source verification from NYS Department of Education
- BLS certification (American Heart Association)
- One of the following wound care certifications:
- CWOCN (Wound, Ostomy, and Continence Nursing Certification Board)
- CWON (WOCNCB)
- CWCN (WOCNCB)
- CWS (American Board of Wound Management)
- WCC (National Alliance of Wound Care and Ostomy)
