Join Presbyterian as an RN Case Manager, providing essential case management in the Emergency Department. You'll enhance patient care and satisfaction, manage resources effectively, and collaborate with an interdisciplinary team to improve health outcomes.
Key Responsibilities
Interviews and assesses each patient, family or other designated person(s) within 48 hours of admission
Applies approved clinical criteria to monitor appropriateness of admissions and continued stays
Communicates with physicians the necessary documentation required to demonstrate medical necessity
Educates patient, family and interdisciplinary team regarding post-acute care options
Develops implements, coordinates, monitors and evaluates discharge plans
Participates and facilitates care progression in daily multidisciplinary rounds
Identifies patients and families with complex psychosocial issues
Documents results of assessments and interventions in the medical record
Required Qualifications
Associate Degree in Nursing
Registered Nurse - State of New Mexico or Compact State Nursing License
Preferred Qualifications
National Case Management certification
Two years clinical nursing experience in relevant clinical practice area with utilization review or case management experience
