Job Description
Join Optum as an Associate Patient Care Coordinator, where you will enhance patient care through scheduling, coordination, and telephonic outreach. This full-time role focuses on supporting Palliative Care providers without direct patient contact, offering a variety of career development opportunities and a comprehensive benefits package.
Key Responsibilities
Provide telephonic patient outreach and patient experience calls
Coordinate patient care as directed by physicians
Support and follow Standard Delegation of Orders (SDO)
Schedule new patient evaluations and follow-up appointments for Palliative Care
Map patient home visit driving routes using web-based applications
Build referrals in EMR systems
Update and maintain master lists and SharePoint
Answer phones and manage medical records
Process requests for medical records release
Work independently and perform other related duties as assigned
Required Qualifications
High School Diploma / GED (or higher)
2+ years of customer service experience with data entry and strong communication skills
1+ years of experience or working knowledge of medical office procedures and medical terminology
1+ years of experience with navigating Internet tools such as Google Maps and Map Quest
Advanced proficiency with Microsoft Office applications (Excel, Word, Outlook)
Preferred Qualifications
Experience in a physician’s office, clinical or hospital setting
Working knowledge of EMR and/or TruCare
Previous experience in a call center environment
Bilingual (English/Spanish) language proficiency
