Job Description

Join Yale New Haven Health as an Acute Care Physician Assistant or Nurse Practitioner, where you'll play a vital role in providing high-quality patient care in a supportive environment that values professional growth.

Key Responsibilities

  • Conduct comprehensive health assessments, including obtaining detailed medical histories and performing physical examinations
  • Develop and implement individualized treatment plans in collaboration with the supervising physician
  • Prescribe medications, order therapies, and manage medical devices according to state regulations
  • Provide patient education and counseling regarding health maintenance and disease management
  • Evaluate and manage common acute illnesses and chronic conditions
  • Perform minor office procedures as trained
  • Collaborate effectively with healthcare team members
  • Document all patient encounters accurately in the electronic health record
  • Participate in quality improvement initiatives
  • Serve as a resource for medical staff and participate in educational activities
  • Provide age-appropriate care to patients across the lifespan
  • Manage referrals to specialists and coordinate follow-up care
  • Maintain current knowledge of family medicine best practices

Required Qualifications

  • 1-2 years clinical experience in primary care/internal medicine setting
  • Active PA or NP in family medicine specialty certification
  • Active, unrestricted CT medical license or in process of obtaining
  • ACLS/BLS/PALS Certification
  • DEA License

Preferred Qualifications

  • 1-2 years clinical working experience as a NP/PA within the given specialty/service line
