• Provide occupational therapy in patients' homes.
• Use Home Care Home Base for documentation.
• Conduct start of care visits and manage a 25•point system.
• Collaborate on patient care plans.

**Qualifications:**
• CA Occupational Therapist license.
• Basic Life Support (BLS) certification.
• 1•2 years of home health or hospice experience.
• Familiarity with Home Care Home Base.
