Position Details:
This is a Home Health RN role covering the Greenwich area. Candidates must have at least 2 years of Home Health experience and be fully licensed at the time of submission. Strong documentation skills are required.

Requirements:

• 2+ years Home Health RN experience
• Experience with OASIS and Homecare Homebase
• Experience with WellSky and wound care preferred
• On-call/tr iage experience
• Reliable, organized, and professional
• Strong time management and customer service skills
• Compassionate, patient-focused approach

On-Call Expectation:

• One to two weeks per month
• On-call hours: 5:30 PM to 8:30 AM , weekdays
